RELEASE AND WAIVER OF LIABILITY
I, the participant or for those under 18, the parent or guardian of the participant, consent to participation in DNOW February 15, 2019 including church provided transportation. I understand the Event will include physical and other activity which involve the risk of accident or serious injury.
I release and forever discharge Oak Hills Church (the “Church") and its respective employees, officers, directors, ministers, elders, volunteers and other representatives and their successors and assigns (collectively “Released Parties”) from any and all claims for injuries, death, losses, or damages the participant might have in connection with the Event. I agree to indemnify, defend and hold the Church and the Released Parties harmless from all damages, including, without limitation, special and consequential damages, suffered as a result of any claims, causes of action, suits, arbitrations, or liabilities incurred or sustained against the Church or the Released Parties related to participation in and transportation to and from the Event.
I UNDERSTAND THAT I AM RELEASING ALL CLAIMS OF ANY KIND ON BEHALF OF THE PARTICIPANT, MYSELF AND ANY OTHER FAMILY MEMBER OR LEGAL REPRESENTATIVE, EVEN IF THE INJURIES, DEATH, LOSS OR DAMAGES ARE CAUSED BY THE NEGLIGENCE OF THE CHURCH OR THE RELEASED PARTIES.
I give the Church and the Released Parties permission to authorize medical and/or hospital treatment for the participant in the event of sickness or injury sustained by the participant.
I grant full permission to the Church to use any photographs, videotapes, motion pictures, recordings, or other records or documents of the Event and to do so without notice or compensation. I assume responsibility for full payment of the cost of the Event; and agree that any and all costs incurred by the participant during the Event, including, without limitation, costs due to health problems, or other emergencies, are my responsibility.
This Release and Waiver of Liability is to be governed by the laws of the State of Texas and is intended to be as broad and inclusive as permitted under Texas Law.
